How do I build a concrete slab?

I want to build a 12x12 concrete slab its only going to be used to anchor metal shelving Industrial w / sheet metal or plastic to the sides and roof, it will be mainly storage, but is also used to work on Small Engine and will therefore be exposed to gasoline, oil, etc. I live in CT and we get some freezes really well here how thick slab should be? Do I need to use rebar? and I can just dig the hole and pour or do I make the stone and sand thing first?

u rebar DONT need, just dig the area .. Make sure that the forms of its level .. built around the area ... and pouring the concrete 4 inches thick ... u might want to rent a mixer if you do it yourself ... thats going to take a few bags of cement
